Who We Serve
We are committed to supporting individuals who face complex and intersecting challenges, including:
- Individuals living with mental illness
- Individuals with intellectual and developmental disabilities (IDD), including those with autism, brain or spinal cord injuries, and chronic medical conditions such as HIV/AIDS
- Individuals who have recently experienced homelessness, often due to disruptions in medical or psychiatric care, including loss of access to critical medications
- Individuals with IDD who are also living with substance use disorders
- Residents of community-based housing environments
- Individuals residing in shared homes with care providers, offering a community-based alternative to institutional settings
Our approach centers on providing wraparound care and coordinated services to promote stability, dignity, and long-term well-being.
